New Patients

My mission is to provide efficient and quality care to improve your quality of life and get you back to doing what you love to do. If you are a new patient or being seen for a new issue, here are some helpful tips to help streamline our care for you during your first visit:

  • Please plan to bring a valid driver’s license and insurance information for check-in.

  • Plan to arrive 30 minutes before your scheduled appointment. This provides time for new patients or patients with new issues to complete intake forms, such as a health survey questionnaire, and complete any X-rays needed prior to evaluation.

  • If you have received imaging from an outside facility (X-rays, CT or MRI scans) that is relevant to your current issue, please bring a copy of the CD with images, as well as the radiologist report.

  • If you have had surgery elsewhere and you are coming in for a related issue or second opinion, please bring a copy of your operative report for my review during your visit.

  • What to wear? If you are being seen for an upper extremity problem, a loose-fitting shirt or tank-top is ideal. If you are being seen for a lower extremity problem, then please bring sweatpants or shorts to change into for your examination.

 

At the end of your visit, you may be asked to complete one or two additional forms relating to your current issue. This information will be used to track your recovery progress and provides feedback for myself to ensure I am providing quality care to all my patients.
